HOW TO USE OUR SERVICES
1. Ensure you are receiving funding for Support Coordination
Support Coordination is not available to all NDIS participants. The funding you receive from the NDIS is based on what is reasonable and necessary for you to pursue your goals.
Support Coordination is included in the Capacity Building Budget and will provide you with a fixed amount for a Support Coordinator to help you understand and use your plan.
2. Get started
Once you have been approved for Support Coordination, you can contact our Support Coordination team on 1800 634 787 or email NDISSupport@swct.com.au.
One of our friendly team members will contact you to arrange an initial appointment that will allow us to get to know you, learn your situation, NDIS goals and identify how we can assist.
3. Personalised Support for the life of your plan
Once your needs have been identified, we will connect you with the providers who will assist you to achieve your goals. We will support you along the journey , build upon your skills and plan accordingly for your NDIS plan review meetings.
